Preparing for your appointment

To help us make the most of your visit, please bring the following:

Photo ID

  • Please bring a valid government-issued photo ID so we can verify your identity at check-in.

Insurance Card

  • Bring your current insurance card so we can verify your coverage and process your visit accurately.

Referral (if required)

  • If your insurance plan requires a referral from your primary care provider, please bring a copy with you or have it sent to our office prior to your appointment.

List of Current Medications

  • Include all prescription medications, over-the-counter medications, vitamins, and supplements you're currently taking to help us provide the safest and most effective care.

Copies of Previous Imaging

  • If you've had X-rays, MRIs, CT scans, or other imaging completed at another facility, please bring the imaging disc and any reports if available. This may help avoid unnecessary repeat imaging.

Records

  • If you've received treatment for your condition elsewhere, bringing relevant medical records, operative reports, or physician notes can provide valuable context for your evaluation.
